New Client Requirements

Effective immediately, all new clients are required to complete a Request for Services form and a Confidentiality Agreement form.  The purpose of the Request for Services form is to help Assistant Services & Associates quickly and clearly identify the services being request and to provide the best services possible.  The form also helps to track all new clients and services provided through monthly reporting.  The Confidentiality Agreement form secures the protection between both Assistant Services & Associates (service provider) and client from third party entities, which prohibits disclosed information received from the client being shared with third parties unless otherwises instructed to do so by law, or by the expressed and implied written permission of the client.  Services cannot be delivered without these documents on file.  Refusal to submit these forms subjects requests for services to be denied.  NOTE:  A letter requesting services is no longer sufficient.

Email requests for services should include complete business and contact information and should describe the type of business.  Requests received via postal mail should be on business letterhead and should include types of services being requested, and signed by the contact person who will be in charge of the decision making process in terms of the services being requested and delivered, and how they are to be rendered.

Nonprofit organizations should submit their requests for services via mail on letterhead signed by the Executive Director or Associate Director (if available) and should include a description of the services requested, type of organization, current operating budget, and a copy of their exempt letter from the IRS to verify they are a 501(c)(3).

All Correspondence (postal mail or electronic) must include a completed and signed Request for Services form and Confidentiality Agreement form in order for services to be reviewed and provided.  Deposits where appropriate will be due upon receipt and approval of agreed services and rates.  You may download a copy of the Service Forms, which includes both the required forms, or you can request a complete packet via email at info@asstservices.com
